Under this agreement, I accept responsibility for any damage, loss or theft that may occur while the loaned equipment is in my possession. I understand that my student account will be charged for the replacement value of any lost equipment or the repair value of any damaged equipment.

 If I need to continue using this equipment past the listed due date/time, I will open an “Equipment Loan Extension” ticket prior to the due date/time to request a 24-hour loan extension. I understand that a loan extension will only be granted if the item is not already reserved by someone else. If my loan extension is denied, I agree to return the equipment on the due date/time listed on the loan agreement form.

 I understand that a $30 late fee will be charged to my student account, should I fail to request a loan extension or return the equipment past the listed due date/time.

 I further authorize ITS to apply any outstanding charges to my account at the Business Office.

 I understand that I will not be eligible to register in courses for future semesters, receive transcripts or diplomas upon graduation until all fees are paid in full.
